Manual assembly or semi-manual assemblies are sources of failure on product as they maintain high level of human involvement. Although there are many studies concentrating on quality control of assembling product, they are lacking of a systematic solutions and holistic view for assembly process. Steven Kmenta proposed assembly FMEA to identify errors on conceptual design and existing products. Tang Xiaoqing (2010) developed a new model for quality assurance in mechanical assembly. Sire (2015) examined the scenarios of failures for riveted assemblies. Hinckley (1994) introduced his quality model concerning three principle sources of assembly failures: Variation, Human error and complexity. Some other researches were also conducted but mainly focus on some certain problems of assembly and have not considered the relationship of influence and disturbance factors to the assembly processes. This research aims to analyze causes and root causes within assembly process. It starts from the failures of assembling products and establish a connection to activities of assembly processes and their influencing factors. The relations and measurements for causes and failures of assembly line are also analyzed to find the most general solutions for the problem. Metal Dolly is a sample product for applications and demonstrations of the findings.en_US
